Custom Foot Orthoses:
Your Path to Improved Mobility
Foot orthotics are medical devices inserted into existing footwear to support, align, or improve the function of the foot and lower limb. They range from simple over-the-counter inserts to highly specialized custom orthoses.
Custom foot orthotics are often prescribed to manage a variety of conditions, including but not limited to plantar fasciitis, metatarsalgia, Morton’s neuroma, heel spurs, hyper pronation, flatfoot deformities, and mildPosterior Tibialis Tendon Dysfunction (PTTD). These devices work by redistributing pressure, improving alignment, and supporting structures. Custom foot orthotics can be fit in variety of shoe types such as athletic shoes, dress shoes, and work boots.
Among custom orthotics, the UCBL (University of California Biomechanics Laboratory) orthosis is a rigid, deep-heel-cup device that encapsulates the heel and midfoot. It is specifically designed to control hindfoot motionand maintain subtalar joint alignment. Unlike softer or more accommodative orthotics, the UCBL is ideal for more severe biomechanical pathologies such as rigid flatfoot, pediatric flexible flatfoot, and moderate to severePosterior Tibialis Tendon Disfunction (PTTD), where maximum control is needed to prevent deformity progression.
What to Expect at the appointment
Initial Appointment
Assessment
This initial stage includes a detailed medical history and physical examination. The clinician evaluates symptoms, foot structure, range of motion, muscle strength, joint alignment, and any existing deformities. Weight-bearing and non-weight-bearing positions are both examined to determine structural versus functional abnormalities.
Biomechanical Gait Analysis
This involves observing and analyzing the patient’s walking pattern. The clinician looks for abnormalities such as overpronation, limited dorsiflexion, limb length discrepancies, or compensatory movements.
Shape Capture
If a custom orthotic is needed, an 3D impression of the feet is taken, typically in a subtalar neutral position.

Fitting Appointment
Once the orthotic is fabricated, the patient returns for a fitting session. The orthotic is placed in the patient’s footwear, and adjustments are made as necessary to ensure a proper fit. The device should hold the foot in the corrected position without causing pressure points or discomfort.
Follow-Up
Follow-up visits are scheduled as needed to ensure optimal comfort and performance of your orthotics. We recommend contacting our clinic for a follow-up appointment if you experience any issues—particularly areas of high pressure, discomfort, or skin irritation.
